Job summary
Visiting Nurse Home Care & Hospice in North Conway, NH seeks a Hospice Director of Patient Services to lead the clinical hospice programs and ensure compliance with federal and state regulations.
You will supervise an interdisciplinary team, manage budgets, collaborate with referral partners, and drive quality improvement while maintaining patient- and family-centered care.
Candidates should hold MSW or RN license in NH/Maine, with 3–5 years in hospice and at least 2 years in leadership.
Qualifications
- Must have MSW or RN license in NH and Maine.
- 3–5 years hospice/palliative care experience.
- Minimum 2 years in leadership or supervisory role.
- Strong knowledge of hospice regulations and reimbursement models.
Responsibilities
- Provide day-to-day operational leadership for hospice services.
- Supervise, onboard, train, and evaluate interdisciplinary team members.
- Ensure safe delivery of high-quality, patient- and family-centered care.
- Monitor clinical outcomes, patient satisfaction, and quality indicators.
- Maintain compliance with federal, state, local, accrediting standards.
- Develop and manage departmental budgets and staffing plans.
- Collaborate with referral sources and internal stakeholders for growth.
- Lead interdisciplinary team meetings and care planning processes.
