Job summary

One Community Hospice seeks a Clinical Liaison to develop relationships with healthcare professionals and the community, coordinating care and promoting hospice services across our Missouri service area. This role reports to the Executive Director and requires a proactive, ethical professional with strong communication and networking skills.

You will develop a territory marketing plan, maintain referral sources, and facilitate admissions by collaborating with clinicians, ensuring HIPAA

Qualifications

• Four-year degree in healthcare, business, psychosocial, or hospice field.
• Active MO RN license required.
• Experience marketing healthcare services or products.
• Hospice, home health, or post-acute care experience preferred.

Responsibilities

• Develop relationships across the community to raise hospice awareness.
• Create and manage a territory marketing plan that meets growth expectations.
• Build relationships with healthcare professionals to facilitate patient access to hospice services.
• Coordinate call schedule and report results to the Community Relations Director.
• Promote hospice services and networking within the healthcare community.
• Collaborate with the clinical team to facilitate admissions to hospice services.
• Admit new patients and support RN visits as needed.
• Ensure HIPAA compliance and regulatory adherence in all activities.
• Represent the organization at community events and maintain relationships with referral sources.
