Bella Terra Home Health and Hospice in Southern California is seeking a Hospice Community Liaison to advance referral development with physicians, hospitals, and other healthcare partners. You will build trusted relationships to promote our compassionate home health and hospice services.

Qualifications

Responsibilities

• Develop and maintain referral relationships with physicians, hospitals, and clinics.
• Conduct regular visits to referral sources to identify patients for hospice services.
• Represent Bella Terra Home Health and Hospice at community events and health fairs.
• Create and implement strategic marketing plans to meet growth objectives.
• Monitor market trends and ensure compliance with healthcare regulations.
