Job summary

CenterCare Hospice in Pueblo, CO is seeking a Community Liaison to support patient and family care across Colorado Springs and Pueblo. The role emphasizes compassionate outreach, liaison with referral sources, and maintaining regulatory and confidentiality standards.

The ideal candidate has hospice liaison/marketing experience and a commitment to quality of care, with occasional travel to serve surrounding areas.

Qualifications

• Must have a minimum of 1 year hospice Liaison/Marketing experience
• Completes annual education requirements
• Maintains regulatory requirements
• Maintains patient confidentiality at all times
• Reports to work on time and as scheduled, completes work within designated time
• Attends annual review and department in-services, as scheduled
• Represents the organization in a positive and professional manner
• Actively participates in performance improvement and CQI activities
• Complies with all organizational policies regarding ethical business practices

Responsibilities

• Represent CenterCare Hospice in the community to attract and assist patients and families.
• Engage with referral sources and community partners to grow service area coverage (Colorado Springs, Pueblo and surrounding areas).
• Coordinate outreach activities while maintaining high standards of patient confidentiality and professionalism.

Salary Range: $70,000.00 - $80,000.00 Salary/year

CenterCare Hospice is looking to add a Community Liaison to their team. Our service area covers Colorado Springs, Pueblo and surrounding areas.

The ideal candidate will be compassionate, empathetic and able to dedicate themselves to our goal of providing the highest quality of care that focuses on the patient's physical, emotional and spiritual needs.

Physical Environment

• Walking/mobility, reaching, bending, lifting, grasping, pushing, pulling and fine hand coordination


• Must be able to perform prolonged or considerable walking or standing


• Must be able to see and hear with or without accommodations


• Must be able to lift up to 50 lbs. without assistance


• Potential exposure to occupational hazards including, but not limited to, infectious/contagious diseases, heat, cold, noise, hazardous substances and uncontrolled home environments


• May have to travel in inclement weather
