Job summary

Willow Branch Hospice seeks a Hospice Community Liaison to serve as the primary representative in assigned communities and referral sources, building relationships with hospitals, physicians, SNFs, home health agencies, and other providers to promote hospice services and referrals.

The role educates patients, families, and partners about hospice eligibility, benefits, and services while supporting organizational growth and market presence through outreach.

Qualifications

  • Community Engagement and Outreach skills to build relationships with providers, organizations, and local partners.
  • Strong Communication and Customer Service abilities to explain hospice services clearly and support patients, families, and referral sources.
  • Understanding of Home Care or hospice/home health environments to discuss care options and coordinate with clinical teams.
  • Experience working with diverse communities and the ability to communicate with empathy and cultural sensitivity.
  • Organizational and time-management skills to plan visits, manage schedules, and document outreach activities accurately.
  • Comfort presenting to groups, facilitating educational sessions, and representing the organization at community events.
  • Preferred experience in hospice, palliative care, home health, or related healthcare or social services settings.
  • High school diploma or equivalent required; post-secondary education in healthcare, social work, marketing, or a related field is beneficial.
  • Valid driver’s license and reliable transportation for local travel in the Chicagoland, IL area.

Role Description

The Hospice Community Liaison serves as the primary representative of the hospice within assigned communities and referral sources. This position is responsible for developing and maintaining strategic relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, home health agencies, and other healthcare professionals to promote awareness of hospice services and facilitate appropriate patient referrals. The Community Liaison educates patients, families, healthcare providers, and community partners regarding hospice eligibility, benefits, and services while supporting organizational growth and market presence.

In addition, the Hospice Community Liaison plays a vital role in connecting hospice services with the community through outreach initiatives, educational programs, and relationship-building activities. The position collaborates with patients, families, referral sources, and interdisciplinary team members to ensure timely access to hospice care and enhance the quality of end-of-life support provided to those served.
