Job summary

LilyCare Hospice is seeking a Hospice Community Liaison to grow our referral network across the greater Albuquerque metro. You will own the territory, build relationships with hospitals, SNFs, assisted living facilities, and physician practices, and drive growth through structured sales calls.

In this role, you will plan territory strategies, deliver community education events, manage a marketing budget, and document activity in our CRM.

Qualifications

• 2+ years hospice experience (preferred).
• Valid driver's license, auto insurance, and reliable transportation for daily travel in Albuquerque area (required).
• Established Albuquerque referral relationships and prior hospice liaison/marketer experience preferred.

Responsibilities

• Own the Albuquerque territory as Hospice Community Liaison; build and deepen referral relationships with hospitals, SNFs, assisted living communities, and physician practices through regular sales calls.
• Execute weekly, monthly, and quarterly territory strategy to grow market share and meet admission goals.
• Plan and deliver in-services and community education events to explain hospice offerings and referral timing.
• Manage the marketing budget as your own, projecting ROI.
• Document activity in the CRM, report progress, and coordinate with clinical team on referrals.

We are hiring a Hospice Community Liaison to grow our referral network and bring our care to more families across the metro.

Duties and Responsibilities

• Own the greater Albuquerque territory as our Hospice Community Liaison — build and deepen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices through consistent, well-planned sales calls

• Execute a weekly, monthly, and quarterly territory strategy — prospecting, account development, and call routing — to grow market share and meet admission goals

• Plan and deliver in-services and community education events that show medical and community groups what hospice offers and when to refer

• Manage your marketing budget like it's your own money, projecting return on what you spend

• Document activity in the CRM, report account progress at staff meetings, and partner with our clinical team on clean referral handoffs

Compensation and Schedule

The Hospice Community Liaison role pays $85,000 - $110,000 per year plus performance bonus (negotiable based on market experience) Full-time, Monday–Friday, with occasional evenings and weekends for community events and referral-source availability. Territory is the greater Albuquerque area, based out of our office at 4811 Hardware Dr NE, Albuquerque, NM.
