Job summary

LilyCare Hospice in Albuquerque is seeking a Hospice Community Liaison to grow our referral network and bring compassionate care to more families across the metro. This role covers the greater Albuquerque area from our office location, with a schedule that includes occasional evenings and weekends for community events.

The position offers a competitive salary with a performance bonus, full-time hours, and autonomy to develop your own territory strategy.

Qualifications

• 2+ years of hospice experience preferred.
• Valid driver’s license, current auto insurance, and reliable transportation for daily travel in the Albuquerque area.
• Established Albuquerque referral relationships and hospice marketing experience are strongly preferred.
• Persuasive, outgoing communicator.

Responsibilities

• Own the greater Albuquerque territory as our Hospice Community Liaison—build and deepen referral relationships with hospitals, SNFs, ALFs, and physician practices through consistent, well-planned sales calls.
• Execute a weekly, monthly, and quarterly territory strategy—prospecting, account development, and call routing—to grow market share and meet admission goals.
• Plan and deliver in-services and community education events to show medical and community groups what hospice offers and when to refer.
• Manage your marketing budget like it’s your own money, projecting return on spend.
• Document activity in the CRM, report account progress at staff meetings, and partner with our clinical team on clean referral handoffs.

Duties and Responsibilities

• Own the greater Albuquerque territory as our Hospice Community Liaison — build and deepen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices through consistent, well-planned sales calls

Compensation and Schedule

The Hospice Community Liaison role pays $85,000 - $110,000 per year plus performance bonus (negotiable based on market experience) Full-time, Monday–Friday, with occasional evenings and weekends for community events and referral-source availability. Territory is the greater Albuquerque area, based out of our office at 4811 Hardware Dr NE, Albuquerque, NM.
