Job summary

Moments Hospice in Madison, WI seeks a dedicated Certified Nursing Assistant to provide compassionate personal care and support under an RN Case Manager. This on-the-road role emphasizes patient comfort, dignity, and teamwork.

You will perform daily living activities, document care in the EMR, and participate in training to grow clinically. A valid CNA license and driver’s license are required, with a comprehensive benefits package offered.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required.
Current CPR certification is preferred.
Excellent customer service skills.
Strong communication skills (written, verbal, active listening).
Problem-solving skills.
Time management skills.
Ability to work in an electronic medical record system.
Ability to lift 50 lbs.
Good understanding of body mechanics.
Strong understanding of professional boundaries.
Valid driver's license is required.
Valid Auto Insurance is required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
