Job summary

Suncrest Hospice is seeking a Certified Nursing Assistant/Home Health Aide (CNA/HHA) to provide compassionate resident care under the supervision of nursing management. You will collaborate with a patient care team to coordinate services in line with assessments and care plans.

The ideal candidate is committed to quality of life, works well in a team, and is willing to drive to patient locations with reliable transportation and an active license and insurance.

Qualifications

• Strong commitment to improving quality of life while partnering with our community organizations in patient care.
• Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and Aide leadership.
• Eager to gain higher level of opportunities such as certifications and further education.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Able to work independently and on your own from a day-to-day basis.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• The CNA/HHA position provides resident care to assigned residents under the direction of a Case Manager/DON.
• The role coordinates proper care with licensed staff according to assessments and care plans.
• Follows company nursing policies, procedures, and safety guidelines.
