Job summary

Heartland Hospice is seeking a dedicated Hospice Aide to provide compassionate, hands-on care to patients in their homes or care facilities across Michigan. You will assist with daily activities, monitor conditions, and support families during end-of-life care with dignity and empathy.

The role requires an active CNA license, CPR certification, a valid driver's license, and at least six months of direct patient care experience. Team collaboration and adherence to safety protocols are essential.

Qualifications

• Active CNA license in the state of employment (Required).
• Current CPR certification (Required).
• Driver's license and reliable vehicle with insurance (Required).
• Six months of direct patient care experience (Required).
• Comfortable working in home settings, hospice facilities, or long-term care environments.
• Demonstrated compassion, patience, and ability to handle emotional situations.

Responsibilities

• Provide direct personal care to terminally ill patients.
• Assist with personal hygiene, grooming, and toileting.
• Support safe mobility, including use of walkers and wheelchairs.
• Help with meal preparation, feeding, and light housekeeping.
• Observe and report changes in the patient's condition.
• Offer emotional and psychological support to patients and families.
• Maintain accurate and timely documentation of visits; attend team meetings.
• Participate in Quality Improvement activities and follow safety protocols.
