Sarah Bush Lincoln invites applications for a Hospice Aide / CNA (Hospice House) covering a 6am-6pm PRN shift. The role delivers personal care and health-related services per Plan of Care, under RN guidance, in a supportive hospice setting.

Responsibilities include ADL assistance, team participation, light meals, documentation, and ongoing education. Illinois residency and CNA credential are required, with a state license driving eligibility.

• CNA or completion of 1st semester of RN Program - Sarah Bush Lincoln.
• Driver's License - State of Illinois.

Mansioni

• Assists in implementing the patient Plan of Care.
• Assists patients with activities of daily living.
• Attends in-services, staff meetings, and continuing education.
• Follows directions from the RN for the patient assigned.
• Keeps environment clean and safe; performs light housekeeping.
• Documents observations and tasks completed or refused.
• Observes and reports changes in patient or family status to supervisor.
• Participates in interdisciplinary team meetings.
• Prepares light meals if included in the plan of care.
• Provides hands-on personal care to patients.
