Job summary

Continuum Hospice is seeking a Full Time Hospice Aide (CNA) to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Responsibilities include following the plan of care, assisting with activities of daily living, maintaining accurate documentation, and supporting families

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered CNA certificate in the state.
• Minimum one year CNA experience in acute or long-term care; hospice a plus.
• CPR certification if state-mandated.
• Driver's license, vehicle insurance, and transportation access.

Responsibilities

• Provide personalized care following the plan of care.
• Assist with personal care and activities of daily living.
• Maintain accurate documentation of patient care.
• Support families and patients with compassion and information.
