Job summary

Generations Hospice is seeking a Hospice CNA to provide compassionate 1:1 patient care in the Oshkosh area and surrounding Wisconsin locations. The role requires flexibility to travel within the territory and a schedule of full-time hours from Monday to Friday, with no weekends or holidays.

Responsibilities include personal care, monitoring patient status, mobility assistance, medication help, emotional support, light housekeeping, and thorough documentation.

Qualifications

• Must meet the state training requirements.
• Minimum six months experience including personal care.
• Must be 18+ years of age.
• Complete 75 hours of classroom and supervised practical training (16 hours must be completed before 16 hours of practical training).
• Possess and maintain current CPR certification.
• Ability to read and follow written instructions and document care given.

Responsibilities

• Provide 1:1 hospice care including bathing, dressing, toileting and mobility.
• Monitor and report changes in patient condition.
• Assist with ambulation, transfers and ROM exercises.
• Provide medication assistance per state regulations.
• Offer emotional support to patients and families.
• Light housekeeping and meal assistance; maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with the hospice interdisciplinary team.
