Job summary

BrightSpring Health Services is seeking a dedicated Hospice CNA to join our Generations Hospice team. This full-time position covers Oshkosh, Omro, Berlin and Ripon with travel throughout the territory as required. Expect 8am-5pm weekdays with no weekends or holidays.

You will provide 1:1 patient care, observe changes, assist with mobility, medications, and documentation, while supporting families in a compassionate environment.

Qualifications

• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Possess and maintain current CPR certification
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy and comfortable providing care to the terminally ill

Responsibilities

• Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning
• Monitor & Report: Observe and report changes in the patient's condition
• Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use
• Medication Assistance: Help with self-administered medications per state regulations
• Emotional Support: Offer companionship and comfort to patients and families
• Light Housekeeping: Change linens, assist with meals, and maintain a clean space
• Documentation: Maintain accurate and timely documentation
• Team Participation: Collaborate with hospice interdisciplinary team
