Bloom Healthcare in Lakewood, CO seeks a Hospice Certified Nursing Assistant to provide compassionate, patient-centered personal care to hospice clients per an interdisciplinary care plan. You will assist with bathing, nutrition, ambulation, and continence care while delivering engaging activities and ensuring safety and satisfaction for patients and families.

Position Profile

This rolewill work with hospice clients to provide personal engagement and enhance quality of life. This role will also provide patient-centered personal care (bathing, continence care, nutrition, ambulation) according to the care plan developed by the interdisciplinary team.

Hospice CNA Responsibilities

• Provides engaging person-centered activities for patients at all levels of physical and cognitive functioning.
• Performs quality individualized personal care.

Hospice CNA Minimum Qualifications

• Current CO C.N.A. licensure
• Current CPR certification.
• Excellent communication and interpersonal skills.
• Solution driven, creative and resourceful problem-solving skills.
• Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
• Proficient computer skills.
