Job summary

TRU Community Care is seeking a Hospice CNA to provide compassionate personal care to patients in homes, facilities, and community settings. You will support comfort, dignity, safety, and quality of life while following each patient’s plan of care.

The role involves regular local travel within the service area and working with an interdisciplinary hospice team. The schedule is full-time, Monday–Friday, 8am–5pm, with a $3,000 sign-on bonus paid within 12 months.

Qualifications

• Current Colorado CNA license required.
• 1+ year CNA experience preferred.
• High school diploma or GED required.
• Valid driver's license, reliable transportation, and proof of auto insurance required.
• Ability to travel locally to patient homes and facilities.
• Must comply with agency health screening and immunization requirements.

Responsibilities

• Provide personal care to hospice patients as outlined in the plan of care.
• Assist with daily care needs while maintaining patient comfort, safety, privacy, and dignity.
• Support patient and family goals, preferences, cultural needs, and spiritual beliefs.
• Assist with transferring, positioning, and mobility using appropriate equipment and safe body mechanics.
• Recognize and report changes in patient condition, symptoms, or care needs to the supervising nurse.
• Support the interdisciplinary team approach through communication and coordination of care.
• Use non-medical comfort measures to support patients near end of life.

Location: Broomfield, Lafayette, & Louisville areas; travel to patient homes and facilities required

JOB SUMMARY

We are hiring a Hospice CNA to provide compassionate, hands-on personal care to hospice patients in their homes, facilities, and community settings. This role supports patients with daily care needs while helping promote comfort, dignity, safety, and quality of life.

The Hospice CNA works closely with the interdisciplinary hospice team and follows each patient's plan of care. This is a patient-facing role with regular local travel throughout the assigned service area.
