Qualifications

• Registered Nurse licensure in the state of operation.
• BSN preferred; accredited nursing program graduate.
• Minimum 2 years of supervisory/management experience.
• Current CPR certification; valid driver’s license and auto insurance.
• Ability to lead an interdisciplinary hospice team with strong communication.

Responsibilities

• Review new patient referrals and determine care needs; assign hospice staff.
• Support and guide clinical staff during home visits, case reviews, and real-time consultations.
• Develop patient care plans, set treatment goals, and prioritize care.
• Lead team case conferences for coordinated, holistic care.
• Conduct chart audits and provide quality-improvement feedback.
• Participate in hiring, onboarding, and training of clinical team members.
• Support staff development and ongoing education programs.
