Qualifications

• Associate degree in nursing with current license.
• Three years of RN practice, hospice experience preferred.
• Knowledge of Hospice Philosophy of Care and ability to use Microsoft Word, Excel, Outlook.

Responsibilities

• Provide clinical oversight for nursing management, documentation, and patient care planning.
• Process workflow for Start of Care to ensure continuity of care.
• Participate in IDG meetings to coordinate and review patient care plans.
