Qualifications

• Associate degree in Nursing is required or equivalent.
• Current Nursing License is required to practice as a Registered Nurse.
• Hospice experience is preferred but not mandatory.

Responsibilities

• Provide clinical oversight for nursing management, documentation, and patient care planning.
• Process workflow after Start of Care visits to ensure continuity of care.
• Participate in IDG meetings to coordinate and review patient care plans.
• Review and adjust SOC and recertification visit schedules as needed.
• Maintain and manage clinical orders within the HCHB system.
• Assist in admissions, discharges, and deaths with proper documentation.
• Ensure timely and compliant documentation per agency policies and regulatory requirements.
• Support data collection for compliance monitoring and quality initiatives.
