The Staff Pad is seeking a compassionate Clinical Nurse Liaison (RN) to represent our hospice program in the Dallas-Fort Worth area. Based in Denton, you will build referral relationships, coordinate patient admissions, and blend clinical assessments with community outreach.

You will work under the Director of Business Development, ensure compliance with Texas BON standards, and support high-quality patient care across hospitals, facilities, and homes.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing.
• Texas RN license and CPR certification.
• Minimum of 2 years of RN clinical experience.
• Hospice or home health experience preferred.

Responsabilités

• Build and maintain relationships with physicians, hospitals and community partners to grow hospice services.
• Serve as the primary RN for General Inpatient (GIP) hospital admissions and routine hospice admissions.
• Evaluate patients for hospice eligibility with physicians and the interdisciplinary team.
• Develop and execute community outreach initiatives; track activity in Salesforce.
• Collaborate with Director of Business Development, Chief Nursing Officer, and the IDT to support quality patient care.
• Ensure compliance with all local, state, and federal regulations, including Texas BON standards.
• Promote the organization's mission, values, and philosophy.
• Perform other duties as assigned.
