Job summary

Care Initiatives Hospice - Des Moines is seeking an experienced Clinical Manager - RN to lead and coordinate high‑quality hospice care for patients across home, long‑term care, assisted living, and hospital settings.

You will oversee clinical operations, collaborate with families and caregivers, and drive compassionate, patient‑centered outcomes while maintaining compliance and service excellence.

Qualifications

• Active RN license in the state(s) where you operate.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse position.
• Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.
• CHPN certification preferred or ability to obtain within 24 months of employment.

Responsibilities

• Provide comprehensive care with compassion, dignity, and respect for patients and families.
• Lead clinical team, coordinate with families, and ensure quality of care across settings.
