Job summary

Qualifications

• Current RN licensure in North Carolina and completion of an accredited nursing program.
• Minimum three years of diverse clinical nursing experience; hospice experience preferred.
• In-depth knowledge of hospice/homecare regulations and documentation standards.
• Ability to organize work, manage case loads, and ensure accurate care documentation.
• Strong written and verbal communication skills and CPR certification.

Responsibilities

• Coordinates case management of all patient services.
• Documentation appropriately for patient care and staff performance.
• Supervises assigned Care staff.
• Assists Administrator as necessary.
• Provides direct patient care including Oasis care planning and staffing coordination.
