Job summary

Mission Healthcare in Salem, OR seeks an experienced hospice/home care nurse leader to oversee clinical operations and case reviews. You will collaborate with clinicians, medical directors, and staff to ensure compassionate, high‑quality patient care across our hospice and home health services.

Qualified candidates will be a registered nurse with current licensure, have prior hospice/home care and management experience, and demonstrate strong communication, assessment, and goal‑oriented care

Qualifications

• RN with current licensure in the state.
• Two years of management or supervisory experience.
• Excellent observation, nursing judgment and communication skills.
• Understands hospice philosophy and pain/symptom management.

Responsibilities

• Receives case referrals and reviews patient information to determine hospice/home care needs.
• Assigns appropriate hospice personnel and confers with medical director as needed.
• Reviews cases via home visits, conferences, and record review to guide clinicians.
• Assists clinicians in establishing goals and plans of care.
• Attends case conferences to coordinate care and interdisciplinary involvement.

Pay range (depending on experience): 125k-135k

Schedule/Shift : Mon-Fri 8am-5pm

Territory/Location: Salem, OR

Responsibilities:

• Receives case referrals. Reviews available patient information related to case, including home visits, todetermine hospice/home care needs. Assigns appropriate hospice personnel to case as needed.Conferences with medical director regarding any questions about an individual’s eligibility forservices.
• Reviews and evaluates each case through a variety of means such as home visits, conferences, recordreview and the services provided by clinicians; discusses and verifies impressions, instructs and guidesclinicians to promote more effective performance and delivery of quality home care services; and isavailable at all times during operating hours to assist clinicians as appropriate.
• Reviews patient’s medical diagnosis, prognosis, medications, procedures, and clinical course.
• Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and indeveloping plan of care.
• Attends case conference meetings with hospice personnel to facilitate coordination of care anddiscussion of interdisciplinary group involvement.

Qualifications:

• Registered nurse with current licensure to practice professional nursing in the state.
• Previous experience in hospice/home care setting and with two (2) years management or supervisoryexperience. Proven ability to work within an interdisciplinary setting.
• Has excellent observation, good nursing judgment and communication skills. Understands hospicephilosophy and issues of death/dying.
• Understands principles of pain/symptom management.
