Job summary

Mission Healthcare in Murrieta, CA is seeking an experienced Registered Nurse in a leadership role to manage hospice/home care teams. You will review cases, guide clinicians, and coordinate care across disciplines to ensure compassionate, high-quality services.

The role requires an active CA RN license with hospice/home care experience and at least two years of supervisory responsibilities. This position supports a mission-driven culture and opportunities for professional growth.

Responsibilities

  • Set priorities and develop care plans with immediate and long-term goals.

Pay range (depending on experience): 125k-135k

Schedule/Shift: Monday-Friday 8am-5pm

Territory/Location: Murrieta, CA

Responsibilities:

  • Receives case referrals. Reviews available patient information related to case, including home visits, todetermine hospice/home care needs. Assigns appropriate hospice personnel to case as needed.Conferences with medical director regarding any questions about an individual's eligibility forservices.
  • Reviews and evaluates each case through a variety of means such as home visits, conferences, recordreview and the services provided by clinicians; discusses and verifies impressions, instructs and guidesclinicians to promote more effective performance and delivery of quality home care services; and isavailable at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and indeveloping plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care anddiscussion of interdisciplinary group involvement.
Qualifications:
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Previous experience in hospice/home care setting and with two (2) years management or supervisoryexperience. Proven ability to work within an interdisciplinary setting.
  • Has excellent observation, good nursing judgment and communication skills. Understands hospicephilosophy and issues of death/dying.
  • Understands principles of pain/symptom management.
  • Knowledge of business management, governmental regulations and CHAP standards.
  • Must hold an active Hospice Certification through a recognized Hospice & Palliative Cred entialing Center (HPCC ), or be willing to obtain the certification and/or required training within 12 months of hire.
  • If disciplinary action has been taken against the individual in the last seven years, they may be ineligible for employment.
