Job summary

Anchor Health in the Livermore/Pleasanton area is seeking a qualified CHHA with hospice experience to provide in-home care to patients. You will assist with bathing, grooming, mobility, meals, and daily activities, while maintaining detailed charts and clear communication with the care team.

Qualifications

• High School diploma or equivalent; CHHA program completed.
• 1–2 years direct patient care experience.
• BLS required; strong verbal and written communication.
• California driver's license with valid insurance; travel required.

Responsibilities

• Completes charting and documentation timely.
• Assists with bathing, personal hygiene and daily activities.
• Assists with transferring and ambulation as needed.
• Assists with prescribed exercises and home-based care.
• Prepares meals and assists with eating as required.
• Assists with toileting and use of bedpans or commodes.
• Supports self-care at home to delay institutionalization.
• Reports changes to Case Manager or Team Manager.
