Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) for the North Team in New Jersey. The role involves direct patient care, including ADL assistance, vital signs, medication reminders, and light housekeeping within the patient’s unit.

Formación

• High School Diploma or equivalent is required.
• Must have a State Certified CHHA license.
• Hospice or home health experience preferred but not required.
• Willingness to travel and have reliable transportation.

Responsabilidades

• Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care, and nails.
• Assists with toileting, positioning, transfers, and turning patients.
• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Performs light housekeeping in patient unit and bed linens as needed.
• Feeds and assists with meals.
• Measures vital signs (temperature, pulse, respirations) as supervised.
• Reports changes to Hospice Nurse and documents on requested forms.
• Assists or reminds patient to take medications per regulations.

Educación

High School Diploma or equivalent
