Job summary

Your Health is seeking a Hospice Chaplain to serve palliative and hospice patients in the Charleston area, delivering spiritual and emotional support to patients and families at homes and in facilities.

The role involves spiritual assessment, counseling, prayer, and meaning‑making, collaborating with an interdisciplinary team, supporting bereavement efforts, and traveling between offices and care sites as needed.

Required: MDiv and ordination; travel and license required.

Qualifications

• Master of Divinity (MDiv) or equivalent theological degree from an accredited institution.
• Ordination or endorsement from a recognized religious organization.
• Clinical Pastoral Education (CPE) certification preferred.
• Experience in providing spiritual care in a healthcare setting, preferably hospice or palliative care.
• Knowledge of diverse religious and spiritual traditions.
• Excellent communication, interpersonal, and active listening skills.
• Ability to provide compassionate care to individuals facing end‑of‑life challenges.
• Sensitivity to cultural and religious differences.
• Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.

Responsibilities

• Conduct spiritual assessments to understand patient beliefs and needs.
• Provide spiritual counseling and support to patients and families.
• Offer prayer, scripture reading, and other rituals as requested.
• Facilitate meaning-making and life review conversations.
• Collaborate with interdisciplinary hospice team to ensure holistic care.
• Participate in team meetings and case conferences.
• Provide spiritual support to hospice staff and volunteers.
• Develop relationships with local clergy and faith communities.
• Conduct presentations and workshops on spirituality and end-of-life care.
• Represent the hospice at community events.
• Provide bereavement support to families and assist with memorial services.
