Alternate Solutions Health Network invites applications for a Hospice Chaplain - PRN at Adena Hospice. This role delivers direct spiritual care to patients, families, and caregivers within an interdisciplinary hospice team.

The chaplain provides spiritual assessment, coordinates care with others, and supports memorial services while upholding hospice philosophy and compassionate communication in an interfaith setting.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
• Experience working with death and dying, individuals/family/caregiver.
• Knowledge of and commitment to hospice philosophy of care.
• Hospice experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Spiritual Care: Assess patients and families; coordinate appropriate services within the interdisciplinary group.
• Administrative: Maintain records of spiritual care services for quality assurance and policy reviews.
• Customer Care: Plan and provide funeral or memorial services as requested.
• Teamwork: Provide education and support to the interdisciplinary group; supervise volunteers when assigned.

As a Chaplain , you will be responsible for the provision of spiritual care services to patients and families/caregivers of hospice, either directly or through coordination of care with other spiritual counselors. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

HOW YOU'LL WORK:

You'll provide direct spiritual care to patients, families and caregivers.

MAJOR AREAS OF RESPONSIBILITY:

• Spiritual Care: Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. Facilitate the development in the interdisciplinary group plan of care to meet identified spiritual needs.
• Administrative: Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
• Customer Care: Provide for funeral or memorial services for patients as requested. Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
• Teamwork: Provide consultation, education, and support to the interdisciplinary group on spiritual care. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
