Job summary

ABL Health Care, LLC is seeking a compassionate Hospice Chaplain to provide spiritual and emotional support to patients, families, and caregivers in East Bay County. You will address end-of-life spiritual needs with dignity and comfort for diverse beliefs.

Responsibilities include counseling, developing care plans with the hospice team, offering grief support, conducting spiritual assessments, leading ceremonies, and coordinating with community faith leaders in Contra Costa County.

Qualifications

• Ordination or certification as a chaplain with endorsement from a recognized faith community or chaplaincy organization.
• Minimum of 2 years of experience in hospice or healthcare.
• Ability to provide non-denominational support to individuals of all faiths or no faith.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide spiritual care and counseling to patients and their families, respecting diverse backgrounds, beliefs, and values.
• Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans.
• Offer grief and bereavement support to families and caregivers.
• Conduct spiritual assessments and document care provided in accordance with hospice policies.
• Lead memorial services, blessings, or other ceremonies as requested by patients or families.
• Support hospice staff with spiritual guidance as needed.
• Build relationships with community faith leaders and organizations to provide additional resources for families.
