Job summary

Paradigm Health in Indiana is seeking a Full-time Hospice Chaplain to cover 1-2 counties surrounding the office. The role requires performing spiritual assessments within 5 days of care start, coordinating with the care team, and serving as a liaison to the spiritual community while addressing patients' emotional and spiritual needs.

Requirements include a Master’s degree in Ministry or Counseling, a valid driver’s license, reliable transportation, and hospice experience is preferred.

Qualifications

• Master's degree or higher in Ministry, Counseling or related field.
• Driver's license required.
• Reliable transportation and insurance.
• One year Hospice experience preferred.

Responsibilities

• Performs accurate and comprehensive spiritual assessments within a 5 day window from the start of care date.
• Coordinates care planning and delivery with physicians, patient/family, and healthcare team to facilitate outcomes.
• Acts as liaison between Hospice and the spiritual community.
• Responds to mental, spiritual, social and emotional needs of terminally ill patients and families.
• Provides non-denominational spiritual counseling per patient/family request.
• Participates in bereavement programs and grief support activities as requested.
• Complies with HHAs policies and all applicable laws and regulations.
