Salus Hospice in Oceanside, CA is seeking a compassionate Chaplain/Bereavement Coordinator to provide spiritual care, grief counseling, and emotional support to patients and families throughout the hospice journey.

Responsibilities include conducting spiritual assessments, counseling individuals and families, attending IDT meetings, developing care plans, providing bereavement follow-up, and coordinating resources with diverse faith traditions.

Qualificações

• Graduate of an accredited seminary or school of theology; appropriate certification in hospital or pastoral ministry, or two years hospice experience preferred.
• Strong communication, listening, and interpersonal skills.
• Ability to provide ecumenical spiritual care with openness and respect.

Responsabilidades

• Complete spiritual assessments within required timeframes.
• Provide individual and family pastoral counseling.
• Attend interdisciplinary team (IDT) meetings regularly.
• Develop, implement, and revise care plans for patients and families.
• Provide spiritual care resources and referrals respecting diverse faiths.
• Offer bereavement support and grief counseling before and after patient passing.
• Coordinate bereavement follow-up and maintain documentation.
• Collaborate with hospice team to ensure compassionate, patient-centered care.
