Job Description
The Hospice Chaplain is responsible for providing spiritual care services to patients and families/caregivers of Moments Hospice, either directly or through coordination with other spiritual counselors.
Essential Job Functions/Responsibilities
- Assess spiritual needs of patients and families/caregivers and ensure timely services.
- Develop individualized care plans by participating in comprehensive assessments.
- Provide direct spiritual care to patients and families/caregivers.
- Support community chaplains and spiritual counselors as a liaison.
- Maintain records of spiritual care services for quality assurance and program development.
- Document services and communication with community spiritual care providers.
- Participate in patient care conferences and interdisciplinary meetings.
- Provide consultation, education, and support on spiritual care to the interdisciplinary team.
- Recruit and develop community chaplains, spiritual counselors, and volunteers.
- Provide funeral or memorial services upon request.
- Support bereavement services for staff, volunteers, and community clergy.
- Assist in supervising spiritual care volunteers.
- Participate in quality improvement activities and perform other duties as required.
Position Qualifications
- Graduate of an accredited seminary or theological school (BD, MDiv, or equivalent).
- Experience with death, dying, and supporting individuals/families/caregivers.
- Ability to work within an interdisciplinary team.
- Comfort with interfaith settings and diverse beliefs.
- Strong networking skills with community clergy and congregations.
- Knowledge and commitment to hospice philosophy.
- Hospice experience preferred.
- Valid driver’s license, insured vehicle, and good driving record.
