Job Description

The Hospice Chaplain is responsible for providing spiritual care services to patients and families/caregivers of Moments Hospice, either directly or through coordination with other spiritual counselors.

Essential Job Functions/Responsibilities

1. Assess spiritual needs of patients and families/caregivers and ensure timely services.
2. Develop individualized care plans by participating in comprehensive assessments.
3. Provide direct spiritual care to patients and families/caregivers.
4. Support community chaplains and spiritual counselors as a liaison.
5. Maintain records of spiritual care services for quality assurance and program development.
6. Document services and communication with community spiritual care providers.
7. Participate in patient care conferences and interdisciplinary meetings.
8. Provide consultation, education, and support on spiritual care to the interdisciplinary team.
9. Recruit and develop community chaplains, spiritual counselors, and volunteers.
10. Provide funeral or memorial services upon request.
11. Support bereavement services for staff, volunteers, and community clergy.
12. Assist in supervising spiritual care volunteers.
13. Participate in quality improvement activities and perform other duties as required.

Position Qualifications

1. Graduate of an accredited seminary or theological school (BD, MDiv, or equivalent).
2. Experience with death, dying, and supporting individuals/families/caregivers.
3. Ability to work within an interdisciplinary team.
4. Comfort with interfaith settings and diverse beliefs.
5. Strong networking skills with community clergy and congregations.
6. Knowledge and commitment to hospice philosophy.
7. Hospice experience preferred.
8. Valid driver’s license, insured vehicle, and good driving record.
