Role Responsibilities
  • Report directly to the Administrator, Executive Director or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
  • Help the patient to maintain good personal hygiene.
  • Assist in maintaining a safe and healthy environment.
  • Prepare and serve light meals.
  • Assist in feeding patients when needed.
  • Assist the patient in ambulation.
  • Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
  • Assist with light housekeeping tasks such as emptying trash and changing linens.
  • Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.
  • Attempt to promote patient’s mental alertness through involvement in activities of interest.
  • Provide basic emotional and psychological support to the patient and household members.
  • Report any change in the patient’s mental or physical condition or home situation to the Registered Nurse and team members.
  • Demonstrate basic care techniques to the primary caregiver.
  • Practice infection control measures and standard precautions.
  • Provide accurate documentation with visit itineraries.
  • Attend staff meetings and participate in quality improvement activities as assigned.
  • Complete documentation in a timely manner as outlined in orientation.
Qualifications
  • Compassionate, kind, and empathetic nature toward patients and families.
  • Comfortable providing care in the patient's home environment.
  • Demonstrate flexibility and composure during crisis support.
  • Certified Nursing Assistant (CNA) license.
  • Six (6) months of relevant patient care experience preferred.
  • Valid driver’s license and automobile insurance.
  • Current CPR certification.
