Role Responsibilities
- Report directly to the Administrator, Executive Director or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient to maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals.
- Assist in feeding patients when needed.
- Assist the patient in ambulation.
- Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
- Assist with light housekeeping tasks such as emptying trash and changing linens.
- Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.
- Attempt to promote patient’s mental alertness through involvement in activities of interest.
- Provide basic emotional and psychological support to the patient and household members.
- Report any change in the patient’s mental or physical condition or home situation to the Registered Nurse and team members.
- Demonstrate basic care techniques to the primary caregiver.
- Practice infection control measures and standard precautions.
- Provide accurate documentation with visit itineraries.
- Attend staff meetings and participate in quality improvement activities as assigned.
- Complete documentation in a timely manner as outlined in orientation.
Qualifications
- Compassionate, kind, and empathetic nature toward patients and families.
- Comfortable providing care in the patient's home environment.
- Demonstrate flexibility and composure during crisis support.
- Certified Nursing Assistant (CNA) license.
- Six (6) months of relevant patient care experience preferred.
- Valid driver’s license and automobile insurance.
- Current CPR certification.
