Responsibilities
  • Report directly to the Administrator, Executive Director, or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
  • Help the patient maintain good personal hygiene.
  • Assist in maintaining a safe and healthy environment.
  • Prepare and serve light meals.
  • Assist in feeding patients when needed.
  • Assist the patient in ambulation.
  • Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
  • Assist with light housekeeping tasks such as emptying trash and changing linens.
  • Assist with treatments as ordered on the aide plan of care and as allowed by state regulations.
  • Promote patient’s mental alertness through activities of interest.
  • Provide emotional and psychological support to the patient and household members.
  • Report changes in the patient's condition or home situation to the RN and team.
  • Demonstrate basic care to the primary caregiver.
  • Practice infection control measures and standard precautions.
  • Provide accurate documentation with visit itineraries.
  • Attend staff meetings and participate in quality improvement activities annually.
  • Complete documentation timely as per orientation guidelines.
About You

Requirements:

  • Compassionate, kind, and empathetic attitude towards patients and families.
  • Comfortable providing care in the patient's home.
  • Flexibility and composure in crisis situations.
  • Certified Nursing Assistant (CNA) license.
  • Six months of patient care experience preferred.
  • Additional experience in hospital or emergency settings advantageous.
  • Valid driver’s license and automobile insurance.
  • Current CPR certification.
