POSITION TITLE: Hospice CNA
REPORTS TO: Director of Nursing
JOB SUMMARY:
The Hospice Aide is responsible for providing hospice services consistent with the mission, vision, and goals of the Agency. The Hospice Aide is a member of the interdisciplinary hospice team who performs planned interventions determined by the RN and interdisciplinary team to meet the physical and psycho-social needs of the terminally ill patient and his/her family.
DUTIES AND RESPONSIBILITIES:
- Assist patient with personal hygiene, elimination, nutritional support/feeding, ambulation and exercise, and medication reminders according to the patient’s needs and nursing orders.
- Provide care consistent with written instructions.
- Provide support for and reinforce the team’s teaching of patient’s caregivers.
- Perform other related supportive tasks as specified by Agency policy.
- Communicate with the hospice nurse regarding services provided, significant findings regarding the patient’s functional status change in care needs, and changes in condition.
- Document the care provided and the patient’s response to care.
- Prepare and submit, as directed, all case documentation and required payroll information according to Agency policy.
- Participate with the interdisciplinary team in the development and implementation of the patient’s and family’s plan of care.
- Provide services within the Agency's catchment area.
- Acknowledge patient rights including privacy and confidentiality.
- Consistently display a professional appearance and conduct in all settings.
- Interact with internal and external customers using a positive and professional approach.
- Assume responsibility and display reliability for the completion of all tasks, duties, communications, and actions.
- Support the mission, vision, and goals of the Agency.
- Participate in orientation and performance improvement activities.
- Participate in in-service education earning 12 contact hours per year.
- Abide by Agency policies and procedures.
- Accept other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT:
- Must have visual and hearing acuity and be able to communicate clearly verbally.
- Must be able to read, write, and speak English fluently.
- May be exposed to minor hazards/communicable diseases.
- Physically active walking, standing, bending, lifting, and climbing stairs.
- Must be able to perform fully assisted transfers, with or without transfer devices.
- Must not have any back lifting/moving restrictions.
- Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
QUALIFICATION REQUIREMENTS: EDUCATION, EXPERIENCE, TRAINING, HEALTH & CLEARANCE
Successful completion of one of the following:
- A training program and competency evaluation pursuant to Federal/State law.
- A competency evaluation program under Federal /State law.
- A state-approved nurse aide training and competency evaluation program pursuant to Federal/State law and is currently listed in good standing on the state nurse aide registry.
- A state licensure program pursuant to Federal/state law.
- NOTE: The aide must NOT have had a continuous 24-month gap in furnishing services for compensation since program completion.
- DMV and criminal background check/OIG Exclusion list/National Sex Offender. Registry/fingerprint card as appropriate. Current CPR, TB, and health clearance.
- Completed skills assessment.
- Successful passage (80%) of the Agency competency exam and core competency checklist.
- Have the orientation and training appropriate to the care of the patient and his/her family.
MACHINES, TOOLS, EQUIPMENT, AND WORK AIDS, including but not limited to:
Wheelchair, sliding board, Hoyer lift, bath/shower bench, 3-in-one commode, bathing aides, gloves, blood pressure cuff, stethoscope, thermometer.

Hospice CNAs may care for patients in these age groups: middle-age and above.
