Position Summary: The primary function is to provide personal healthcare and related services to patients in their residence, assist in maintaining a safe and clean environment, work cooperatively with patients and families, and share observations with supervisors. Works under the supervision of a registered nurse.

Qualifications:

• High School Diploma preferred
• Successfully completed nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry, or meet state licensure requirements

• Must have a current driver’s license and reliable transportation
