Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice Certified Nursing Assistant (CNA) to provide paraprofessional home care services under the guidance of a primary nurse and physician. You will work during the day shift with consistent, predictable hours and a focus on compassionate patient care.

Requirements include Michigan CNA certification, reliable transportation, valid driver’s license, and the ability to provide proof of car insurance and vehicle registration.

Qualifications

Responsibilities

• Work under supervision of the primary nurse and comply with Nurse Practice Act and regulations.
• Deliver compassionate, patient-centered care in a home setting.
• Collaborate with nursing staff to ensure continuous patient care and safety.

Description

As a Hospice CNA you will provide paraprofessional services to the organization’s home care clients as developed by the primary nurse and as prescribed by the physician and in compliance with the state’s Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.

Minimum Qualifications
