Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice CNA to provide paraprofessional services to home care clients under the supervision of the primary nurse, in compliance with the state Nurse Practice Act.

This part-time day shift (8am-5pm EST) role offers meaningful patient care, predictable workloads, and opportunities for growth within a nurse-led team. Benefits begin on the first day of employment.

Qualifications

• CNA certification in Michigan is required.
• Reliable transportation and a valid driver’s license.
• Must provide proof of car insurance and vehicle registration.
• High school diploma or GED.
• Previous hospice, palliative care, or home health experience is preferred.
• Compassionate, patient-focused, and able to provide emotional support to patients and families.

Responsibilities

• Provide paraprofessional services to home care clients as developed by the primary nurse and in line with physician orders and state Nurse Practice Act.
• Support patients with activities of daily living and emotional support as part of compassionate hospice care.
• Ensure care is delivered in a safe, compliant, and respectful manner within the home setting.
