Job summary

Elara Caring in Mentor, OH seeks a Certified Nurse Aide to join our home health team delivering compassionate, patient-centered care in patients' homes.

You will provide hands-on personal care, assist with daily activities, monitor vital signs, and follow each patient’s care plan under RN supervision. A valid Home Health Aide or CNA certification and ability to lift up to 50 pounds are required. Valid driver’s license or reliable transportation is needed.

• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Provide hands‑on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes in a patient’s condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
