Details

The Home Health Aide (HHA) position works under the direction of a Case Manager or Director of Nursing (DON) and is responsible for providing resident care to assigned residents in accordance with resident assessments and care plans, as well as directives from nursing management. The HHA collaborates with other licensed care staff to coordinate appropriate care and strictly follows all company nursing policies, procedures, and safety guidelines.

Qualifications
  • Strong commitment to improving quality of life and partnering with community organizations in patient care
  • Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and HHA leadership
  • Eagerness to pursue higher certifications and further education opportunities
  • Ability to work closely with patients on a one-on-one basis while receiving support from a dedicated care team
  • Ability to work independently on a daily basis
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance
  • Home Health Aide certification in the state(s) of practice
