Cantex Continuing Care Network's Elysian Hospice in San Antonio, TX is hiring a Certified Nursing Assistant (CNA) to provide compassionate personal care to hospice patients in their homes or facilities. Under supervision of a nurse, you’ll assist with ADLs, promote comfort and safety, and report changes to the clinical team.

The ideal candidate holds a current CNA certification, has strong communication skills, and can work independently in home or facility settings.

Qualifications

• Current state CNA certification required.
• Hospice, home health, or long-term care experience preferred.
• Strong communication and patient care abilities.

Responsabilités

• Provide personal care including bathing, grooming, dressing, toileting, and mobility assistance.
• Assist patients with activities of daily living per the plan of care.
• Document patient care and observations accurately and timely.
• Report changes in patient condition to supervising nurse.
• Promote patient safety, comfort, dignity, and independence.
• Maintain a clean and safe patient environment and follow infection control procedures.
• Assist with safe transfers and mobility using proper lifting techniques.
• Attend in-service training and maintain CNA certification requirements.
• Communicate effectively with patients, families, nurses, and interdisciplinary team members.
• Follow company policies, procedures, and patient care standards.

Job Summary

The Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their place of residence in accordance with the patient’s plan of care. The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety. The CNA plays an important role in supporting hospice patients and families by assisting with daily care needs and reporting changes in patient condition to the clinical team.

Qualifications

• Current state certification as a Certified Nursing Assistant (CNA) required
• Hospice, homehealth, long-term care, or healthcare experience preferred
• Strong communication and patient care skills
• Ability to follow patient care plans and instructions from nursing staff
• Ability to work independently in a home or facility setting
• Reliable transportation, valid driver’s license, and current auto liability insurance
• Ability to read, write, and communicate effectively in English
