Job summary

Aura Health is seeking a Hospice Care Coordinator to manage and coordinate comprehensive home-based hospice care for assigned patients, ensuring compassionate, compliant support throughout the hospice journey.

You will conduct patient assessments, develop individualized care plans, provide skilled nursing and symptom management, and educate caregivers—working with an interdisciplinary team to deliver high-quality, empathetic care.
