About the Role

The Registered Nurse Case Manager (RN Case Manager) will provide direct patient care services to hospice patients and supportive care to the patient’s family; manage an interdisciplinary group (IDG) consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care; and case manage all assigned patient clinical records.

Schedule: M-F 8-5

Role Requirements

• Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.
• One (1) year of experience in oncology, long term care, hospice or home health setting.
• Current Cardiopulmonary resuscitation (CPR) certification.
• Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
