Elliot VNA is seeking a Registered Nurse for hospice care in Manchester, NH. This per diem, part-time role focuses on delivering compassionate end-of-life care, coordinating with an interdisciplinary team, and supporting families in managing symptoms and goals of care.

You will assess needs, implement plans, educate families, and document progress in the patient record. Prior hospice experience is preferred, with a commitment to high-quality, patient-centered care.

Qualifications

• Must hold an active RN license and be eligible for NH licensure.
• Experience in hospice or palliative care preferred.
• Per diem/part-time availability and reliable transportation required.
• Strong communication with patients, families, and the care team.

Responsabilités

• Deliver home hospice nursing care to patients and families.
• Assess patient needs and implement care plans using nursing process.
• Coordinate with interdisciplinary team for seamless care across shifts.
• Educate patients and families on care, medications, and symptom management.
