Job summary

Northwell Health in Manhasset, NY is seeking a Hospice Care Registered Nurse to provide comprehensive field-based nursing care. You will assess, plan, implement and evaluate care for patients in home settings, and supervise aides in the patient’s home.

The role requires NYS RN licensure and 2+ years of nursing experience in med/surg, hospice, or home care; BSN preferred. On-call coverage and quality improvement participation are expected.

Qualifications

• Graduates from an accredited School of Nursing; BSN preferred.
• Current NYS RN license is required; specialty certifications as needed.

Responsibilities

• Conduct pre-admission screening and initial nursing health assessments on new patients.
• Develop nursing plans of care and goals based on patient needs.
• Provide nursing care including supervision of diet, medication, and activities of daily living.
• Evaluate the provision of nursing care, including therapy and nutrition counseling.
• Train and evaluate home health aides and personal care workers in the patient's home.
• Recommend the rental or purchase of medical equipment and supplies for patients.
• Provide ongoing evaluation of equipment for defects and repairs.
• Participate in on-call coverage system on a rotating basis.
• Participate in the ongoing Quality Improvement Program.

Requirements

• Education: Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Experience: 2+ years of nursing experience in med/surge, hospice, or home care preferred. Minimum one year of experience in acute care – med/surg strongly preferred.
• Licenses & Certifications: Current License to practice as a Registered Professional Nurse in New York State required. Specialized certifications as needed.
