Job summary

Northwell Health is seeking a Registered Nurse to deliver age-appropriate care in a field-based hospice/home health setting. You will perform comprehensive nursing assessments, develop care plans, and coordinate therapies for designated patients.

The role requires New York State RN licensure, with experience in med/surg, hospice, or home care preferred, and flexibility for on-site visits across care sites. Includes collaboration with interdisciplinary teams and ongoing quality improvement

Qualifications

• Graduate from an accredited School of Nursing and provide nursing care.
• Must be enrolled in BSN program within 2 years and obtain BSN within 5 years of entry date.
• Current NYS RN license required, with specialized certifications as needed.

Responsibilities

• Conduct pre-admission screening and initial nursing health assessment; develop care plans and goals.
• Provide nursing care including supervision of diet, meds, activity, safety, and daily living activities.
• Evaluate care by coordinating therapies and services; support rehab and nutrition counseling.
