Job summary

Cadre Family Companies seeks a dedicated Hospice Care Assistant (CNA) to support our mission of delivering exceptional care in patients' homes. You will work with a collaborative hospice team to provide direct patient care, comfort, and dignity, while documenting care and adhering to the plan of care.

The role requires dependable, compassionate individuals who can travel within the service area and interact closely with patients, families, and staff to ensure high-quality end-of-life care.

Qualifications

  • Current Ohio CNA certification.
  • Minimum of one year of CNA experience in hospice, palliative care, or similar setting.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the assigned service area.
  • Strong communication, teamwork, and interpersonal skills.
  • Compassionate, patient-focused approach to care.

Responsibilities

  • Provide compassionate patient care including activities of daily living.
  • Assist with positioning, transfers, ambulation, and mobility needs.
  • Offer companionship and emotional support to patients and families.
  • Respect patient dignity, privacy, and routines.
  • Promote independence and comfort.
  • Observe patient condition and communicate concerns to the RN.
  • Maintain a clean, safe patient environment.
  • Follow infection prevention and privacy standards.
  • Complete accurate documentation of care provided.

About the Role

As a Hospice Care Assistant (CNA), you'll play an important role in supporting our mission of providing exceptional care and service. You'll join a collaborative team of professionals who are passionate about making a difference every day.

What You'll Do

Provide Compassionate Patient Care
  • Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene.
  • Provide assistance with positioning, transfers, ambulation, and mobility needs.
  • Offer companionship and emotional support to patients and families.
  • Respect each patient's dignity, privacy, preferences, and individual routines.
  • Encourage independence and provide care in a way that promotes comfort and confidence.
Promote Comfort & Quality of Life
  • Provide comfort measures according to the patient's individualized plan of care.
  • Observe patients for changes in condition, behavior, pain, or comfort level and promptly communicate concerns to the supervising nurse.
  • Assist with creating a peaceful, safe, and comfortable environment.
  • Provide compassionate presence and reassurance to patients and loved ones throughout the end-of-life journey.
  • Work closely with the RN case manager and interdisciplinary hospice team to support individualized patient care.
  • Communicate changes in patient condition or needs to the appropriate member of the clinical team.
  • Build trusting relationships with patients, families, caregivers, and members of the hospice team.
  • Follow the patient's plan of care and perform duties within the scope of your certification and role.
  • Assist with maintaining a clean, organized, and safe patient environment.
  • Change linens and assist with basic household tasks related to patient care as appropriate.
  • Follow infection prevention, safety, and patient privacy standards.
  • Complete accurate and timely documentation of care provided.

Required Qualifications

  • CurrentCNA certificationin the state of Ohio.
  • Minimumofone year of experience working as a CNAin hospice, palliative care, or a similar healthcare setting.
  • Valid driver's license and reliable transportation.
  • Clean driving record and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong communication, teamwork, and interpersonal skills.
  • Compassionate, patient-focused approach to care.
