Summary

We are hiring for a Hospice Aide / CNA.

This is a PRN/Inconsistent position doing Home Visits for Hospice.

  • Weekday daylight availability required.
  • STNA required.
  • Hospice or Home Health experience required.

Responsibilities

The Hospice Aide provides personal direct patient care services under the supervision of a registered nurse, as established in the Plan of Care.

  • Reports after patient encounters to the Executive Director or designee regarding the patient's condition or environment, following the Care Management process as needed.
  • Assists with ADLs such as mobility, transfers, walking, grooming, bathing, dressing, undressing, eating, and toileting during each patient visit.
  • Performs incidental household services essential to the patient's health care at home, conducting household services necessary to prevent or postpone institutionalization.
  • Participates in QAPI (Quality Assessment & Performance Improvement) activities, identifying problems and reporting to the administrator as needed.
  • Must have reliable transportation.
Education and Experience

License Requirements

  • Successful completion of a competency evaluation prior to patient care.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Additional State Specific Requirements

  • OH: Successful completion of a state-approved training and competency evaluation program, and registration in the state nurse aide registry.
Skill Requirements
  • Mature and able to handle the demands of a hospice aide position.
  • Ability to read, write, and follow directions accurately and promptly.
