Job summary

TriHealth is seeking a Hospice Aide I to support Hospice of Cincinnati patients in homecare/long-term-care, covering an East territory with a touch base office in Anderson. The role is full-time, Monday through Friday, 8:00 AM to 4:30 PM, with mileage reimbursement and field-based work in patients' homes.

The ideal candidate will have a High School Diploma or GED, 6 months to 2 years healthcare experience, BLS within 30 days, and reliable transportation to serve patients in the eastern Hamilton,

Qualifications

• BLS certification within 30 days is required.

Responsibilities

• Performs and documents basic patient care tasks under nurse supervision, including bathing, feeding, ambulation and companionship.
• Communicates effectively with the patient’s inter-disciplinary team and family members.
• conducts quality checks on equipment and supplies and reports issues.
• Monitors and restocks supplies in the patient care area.
• Follows safety policies and regulatory documentation requirements.

This position works full-time Mon-Fri dayshift for the Hospice of Cincinnati East Home Care/Long term Care team with a touch base office in Anderson. This is a very large East territory of Cincinnati for hospice patient care in homecare/long-term-care. We cover the eastern parts of Hamilton County, and Clermont, Brown and Warren County. Must be available to work Mon-Friday days, no weekends or holidays. Care is provided in the patient's home or in their home/long-term-care home, so must have reliable transportation. HOC mileage paid.

Work Schedule:

• Full Time (40 hours weekly)
• Day Shift 8am-4:30pm (No weekends or holidays)

Job Overview:

The Hospice Aide I provides direct patient care to Hospice of Cincinnati patients under the direction of a nurse. The hospice aide will assist with, and/or perform basic activities of daily living, including bathing, feeding, assistance with ambulation, etc., and will serve as a provider of companionship and emotional support, even during times of crisis. The position is responsible for knowing safety policies and following approved procedures.

Job Requirements:

• High School Diploma or GED 2 years of high school education in general studies (Required)
• 6 months- 2 years Previous Healthcare Experience (Required)
• Reading and Math ability appropriate to the position.
• Basic Life Support (BLS) within 30 Days Required

Job Responsibilities:

• Performs and documents basic patient care tasks under the direct supervision of a nurse, including bathing, feeding, assistance with ambulation, providing companionship and emotional support, even during times of crisis; follows regulatory and organizational processes and practices to promote a safe clinical environment.
• Communicates effectively with the patient’s interdisciplinary team, including patient and patient family members.
• Performs equipment and supply related quality checks and reports problems/malfunctions appropriately.
• Monitors supply rooms, reports missing supplies, restocks supplies as needed.
• Complies with regulatory and organizational documentation requirements to ensure accurate reporting of clinical interactions and patient status; ability to demonstrate a basic understanding of technology for documentation purposes.
