Overview

Responsibilities

POSITION SUMMARY: Each Home Health Aide enhances the ability of Tabitha's Seniors/residents/clients to remain at home, living independently for as long as possible by providing personal care, emotional support, assistance with activities of daily living, and basic therapeutic care. They contribute to the physical, mental, emotional, and spiritual well-being of Seniors/clients through respectful, professional, competent, and personable interaction. This role includes maintaining accurate records of care and supporting Tabitha's LIVE2Care culture and Core Values. The aide collaborates with family, staff, management, volunteers, and others to ensure quality care.

Essential Functions
  1. Performs personal care and therapeutic activities according to the care plan.
  2. Identifies educational needs for Seniors/clients and communicates with the nurse.
  3. Provides care in various settings as assigned.
  4. Monitors use of equipment and reports issues.
  5. Offers emotional support to Seniors/clients and families.
  6. Observes and reports changes in condition.
  7. Documents care and communications accurately.
  8. Follows the Senior's/client's Bill of Rights and organizational policies.
  9. Prioritizes and adjusts tasks to meet changing needs.
  10. Drives between locations, adhering to safety and road conditions.
  11. Maintains confidentiality and complies with HIPAA.
  12. Participates in training, safety, and quality activities.
  13. Maintains professionalism, punctuality, and team collaboration.
Additional Functions

For Hospice aides: Provides meal prep, household duties, and other supportive tasks.

For Home Health Medication Aides: Assists with medication administration per policy (Note: Hospice aides do not administer medications).

Qualifications

Communication: Fluency in English, including medical terminology, ability to follow instructions, document care, and communicate effectively.

Worksite Conditions: Indoor and outdoor work, exposure to various environmental factors, extensive driving, and potential for emotional interactions.

Physical Requirements: Ability to perform physical tasks, transfers, and respond to emergencies.

Equipment Usage: Familiarity with standard and specialized medical equipment.

Age, Education, Experience: Minimum 18 years old, high school diploma preferred, home health aide or nursing assistant experience preferred.

Credentials: CNA certification in good standing, CPR certification, valid driver’s license, auto insurance, and compliance with vaccination requirements.
